To Enroll your child please follow the steps below:

1- Download and fill out the liability waiver and registration form.

2- Answer Questions Below with your child's information. Next, Attach waiver and registration forms on the link that says "click here". Press "send" once completed.

3- Pay in full by sending a Venmo payment to Dance and Play (@Katy-Gradillas) with your child's name, service, and location.

(We will not be able to refund a session after the First Week)
